School Year 2024-25
Welcome to Washington State University Athletics! WSU Sports Medicine coordinates medical care to student-athletes and spirit squad members for injuries and illness they encounter while competing for WSU. WSU Sports Medicine will be financially responsible for student-athlete sport-related injuries and illnesses and spirit squad members participation injuries, occurring as a direct result of official team activities. WSU Sports Medicine will be the secondary payer after the student-athlete’s/spirit squad member’s primary and secondary health insurance are processed to pay for the health care services. In the event a student-athlete/spirit squad member is insured by a federal health care program (e.g., Medicaid, TriCare) or certain limited health plans that expressly exclude collegiate sport injuries/illnesses, WSU Sports Medicine will be the primary payer. WSU Sports Medicine will also cover co-pays, co-insurance, and deductibles where the student-athlete’s/spirit squad member’s personal insurance is the primary payer. Student-athletes and spirit squad members will NOT be able to practice until they have been cleared by the WSU Sports Medicine staff.Â
Below is a list of forms to be completed and returned to WSU Sports Medicine. With the insurance paperwork, please include a clear copy of the front and back of your Health Insurance (primary & secondary), Prescription Drug, Dental, and Vision cards. See below for options to return your paperwork. Â
These packets differ depending on your participation status – Please make sure to select the paperwork packet that applies to you. Student-athletes/spirit squad members that are NOT 18 years of age when the paperwork is completed must have their parent/guardian sign all the paperwork to be accepted by WSU Sports Medicine.Â
All WSU Student-Athletes:

- All students are encouraged to have insurance. All International students with F1 and J1 visa status who are enrolled at WSU are automatically enrolled in the WSU "iSHIP" (International Student Health Insurance Plan)
- Receive an athletic-related medical bill and/or your medical insurance changed this year?
